In order to ensure that our certified technicians can replace or repair your handpieces as quickly and as thoroughly as possible, we’ll first need to collect some information. By providing us with as many details as possible, you can help ensure that we are able to provide you with the superior, efficient and personalized high speed, slow speed and electric handpiece repair services that you deserve.

1. If you are a new client to DentX, click here to download the New Client form. Once completed please email to info@dentxsolutions.com. **Not applicable for previous Militech clients.

2. Click here to print and fill in the prescription form for your handpiece repair and submit it with your handpiece.

3. Fill in the pick up request form, on this page, so we can schedule a pick up on your behalf. If a quote is necessary prior to repair, please specify in the notes section.
